Obtaining Services from Horse Healing

We accept most insurance plans including commercial insurance, Tricare and Medicaid.  We will be happy to bill your insurance directly.

You must obtain a referral, usually from your Primary Care Provider, to begin the process.  Your referral must state the diagnosis or issue that you are seeking to have treated.

If you have Medicaid , your referral must come from your Primary Care Provider, the provider listed on your Medicaid card.  Your referral must include diagnosis or issue to be treated.

You will need to send copies of your insurance card, both sides.  Please also send your name, address and phone number for the billing department to use if they have questions.    If this is a minor, please send the information pertaining to the parent or guardian.

Your information can be faxed directly to our billing department at 833-694-1490.  Please make sure to designate this is for Horse Healing.  You can also email your information to Spiritstablesflorida@gmail.com and it will be forwarded.  

Your information will be submitted in order to obtain an authorization for services.  This could take up to 5 to 7 business days depending on your insurance plan.
